What actually changes

Fewer suppliers. Frequently one, sometimes two. Your negotiating position depends entirely on knowing this before the conversation starts.
Regional players matter more. The national carriers advertise coverage; the local operator often owns the fiber that actually passes the building. National brand recognition is a poor predictor of who can deliver.
Longer lead times, mostly because the crews and the equipment are further away.
Backhaul becomes visible in the price. In São Paulo the distance to a major interconnection point is negligible. Eight hundred kilometres from one, it is a line item.
Support distance. The nearest technician may be in another city, which turns a repair commitment into a driving time.

Where wireless stops being second best

In a secondary city, a licensed point-to-point radio link is often not the compromise — it is the fastest path to a working circuit, and sometimes the only one inside a usable timeframe. It needs line of sight and it is affected by heavy rain, and both of those are manageable engineering problems with known answers. Waiting six months for fiber that may not arrive is not.

Questions worth asking early

How many carriers can genuinely deliver here, as opposed to how many claim coverage?
Where does the backhaul from this city terminate, and how many hops to the nearest major interconnection point?
Where is the nearest technician, and what is the committed time to be on site at 3am on a Sunday?
Is there any second path at all, and does it share a tower or a duct with the first?

Why a neutral view helps more here

In a capital, comparing suppliers is a convenience. In a secondary city it decides whether the project happens at all, because the answer is often one specific regional operator that a national procurement process would never have found.
